FLOWCHART - INTAKE PROCESS
While working as Manager of the Speech-Pathology Department, I identified referral error as a high-risk issue. To address missing, incomplete, or duplicated referrals, I created this flowchart depicting the Child Development Team Referral Intake Process. Once this flow was implemented, the referral error rate dropped from 55% to <5%.
